Ordering Glucose Meters and Supplies

Medicare

1. Patient needs prescription for meter and strips.
2. No restrictions on type of meter, but can only fill a script for 25 tests strips per month unless specified on prescription why must test more often.
3. Call Patient Care Services at 1-800-258-7839 with patient information, fax them prescription at 846-3211. * They will make a home visit to instruct patient in meter use, * they will mail supplies on a monthly basis.
4. Patient can fill prescription at any participating pharmacy, but many do not accept assignment and patient must pay up front then wait for reimbursement.

Medical Assistance

1. Patient needs prescription for meter and strips.
2. No restrictions on type of meter or amount of strips at present. Put on prescription amount of strips needed for testing pattern.

Blue Cross Classic, Straight Healthmate, Lifespan Blue, Mass. Health, and any Private Insurance

1. Patient requires prescription for meter and strips.
2. No restrictions on type of meter or amount of strips, order amount of strips appropriate for testing pattern.
3. Call Patient Care Services at 1-800-258-7839 with patient information, fax them prescription at 846-3211. * They will make a home visit to instruct patient in meter use, * they will mail supplies on a monthly basis.

BlueChip, Healthmate 2000, and Healthmate Coast to Coast, and State Blue

1. Patient must have prescription for meter and strips.
2. Must call/fax information to Vanguard at 468-1300 or 1-800-696-3000, fax # 468-1333. Vanguard stocks the following meters: Glucometer Elite, Accucheck Advantage, Precision QID, or One-Touch, but you can request any meter at present. Vanguard is the preferred provider so must be used.
3. If patient has never had a meter through Vanguard before, Vanguard must obtain prior approval so there may be a slight delay in obtaining supplies.
4. Vanguard will mail/ship supplies to patient.s home.

United Healthcare/ United Senior Care

1. Patient must have prescription for meter and strips.
2. Meters available: All models available depending on supplier.
3. Patients have choice of two providers to obtain supplies:
   1. South County Surgical Supply, tel.# 783-1850 fax#783-2082, 344 Main St., Wakefield, RI 02879. They stock all models.
   2. Kent County Home Medical Equipment, tel.# 736-4294 fax# 736-1006. They only stock the Accucheck Advantage model.
